To: Parent of a Kindergarten Student for the 2020-2021 school year
If you have a child who will be five (5) on or before August 31, 2020 and will be attending kindergarten in USD 495 next fall, please complete the digital Kindergarten Pre-Registration Form. If you are unable to complete the digital version, we will have forms at lunch pick-up locations starting Tuesday.
The following items are required to be on file with the school at the time of the child’s enrollment:
Kansas Certificate of Immunization
Official Birth Certificate
A Kindergarten informational meeting will be scheduled at a later date. Thank you for your patience.
